What Is Physiotherapy?

Physiotherapy is an evidence-based service that helps improve functional capacity. Physiotherapists at Therapy Circles Australia will collaborate with you to determine what goals you would like to achieve from therapy and how this will be carried out. A key role of a physiotherapist is to conduct a thorough mobility and physical assessment to determine what impairments may exist and how to best address these with therapy. 

Physiotherapists can deliver exercise therapy sessions, targeting the identified impairments to help improve your functional capacity and ability to perform activities of daily living. This can be achieved in several ways, such as home-based exercises, gym exercise sessions or aquatic therapy sessions, and will be tailored to suit your individual needs. There is strong evidence to support the effectiveness of physiotherapy in helping to improve muscle strength, joint range of motion, balance and posture. Physiotherapists are also trained in providing pain management, by utilising exercise therapy, hands-on skills (e.g. soft tissue massage and joint mobilisations) and education on pain-reducing strategies.

Physiotherapists at Therapy Circles Australia will work closely with you, your support workers and support networks to implement mobility strategies to help provide you with the best quality of life in the long term. Our physiotherapists can help to obtain assistive technology that will facilitate your goals and capacity to perform tasks of daily living.

We will tailor our Physiotherapy service to help facilitate your individuals needs and goals. We are committed to providing a collaborative therapy treatment plan with you and your support networks. 

The following is a guide of what our Physiotherapy service entails:

Initial Assessment:

  • We will work with you and your support networks to collaboratively establish goals of physiotherapy and clarify what you are wanting to achieve. This will be based on the evidence-based principle of SMART goal setting (specific, measurable, achievable, realistic and have a set time frame). Your individual therapy goals will be monitored routinely throughout the physiotherapy sessions.
  • The physiotherapist will conduct a thorough mobility and physical assessment to understand what areas to address with therapy and how to aid achievement of your goals.

Therapy Intervention Sessions:

  • Therapeutic strategies that are used will be based on your individual needs and goals. The following is a list of treatments our physiotherapists can provide:
    • 1 on 1 exercise therapy sessions in the home
    • Creating and implementing a home-exercise program
    • Aquatic or hydrotherapy exercise sessions
    • Gym-based exercise sessions
    • Pain management and education
    • Collaborating with you, your support workers and support networks to provide mobility strategies to help you perform activities of daily living.
    • Falls prevention and balance training
    • Gait re-training
    • Cardiopulmonary rehabilitation
    • Prescription of assistive technology
    • Provide training to your support networks on how to help you optimise mobility and function in the home and the community.

Ongoing Plan Monitoring and Review:

  • We implement evidence-based assessment tools and outcome measures to regularly monitor the progress and effectiveness of our therapy interventions. This allows us to make appropriate adjustments to the therapy plan as required.
  • Our physiotherapists will work collaboratively with your support networks and the whole multidisciplinary team to provide ongoing progress reviews and deliver a well-rounded care program for you.
  • We explore barriers that may present themselves and help to problem solve these with you and your support network.
  • We welcome and encourage your feedback at all stages of the service provided.

Before our Initial Meeting:

  • Our Physiotherapists will review all documentation and information you have provided us (NDIS plan and goals, referral and service handover information).
  • Physiotherapists will research and refer to evidence-based guidelines to plan therapy strategies tailored to your diagnosis.
  • Physiotherapists will arrange an initial meeting at a time that is most convenient for you. Attendance of your key support network is also encouraged.

Initial Assessment:

  • Discuss the aims of Physiotherapy and frequency of the service in accordance with your specific needs and availability of funding.
  • Collaborative goal setting to guide the therapy plan.
  • Collectively discuss therapy strategies and interventions that will benefit you and your goal achievement.
  • Physiotherapist will conduct a mobility and physical assessment. This will serve as a baseline reference that can be regularly re-tested and analysed, to measure the changes we are making.

Ongoing Therapy Intervention Sessions:

  • Delivering the therapy treatment plan in accordance with your specific needs and evidence-based practices (please refer to list of potential treatment strategies stated above).
  • Regular monitoring and re-testing of physical and functional outcome measures that will help track treatment and goal progress. Appropriate changes and modifications to the treatment plan can be made in accordance with this.
  • Collaborating and communicating with all involved including family and support workers to help facilitate physiotherapy treatment strategies on a day to day basis.

Physiotherapist to complete NDIS Plan Review Report:

  • Plans are required to be reviewed every 12 months.
  • Plans may be required to be updated/amended prior to the end date. Physiotherapists can provide support letters if the plan requires amendment before the end date.
  • Physiotherapists will advocate for ongoing supports and requirements that you may need in accordance with your NDIS and therapy goals.

Initial Physiotherapy Assessment
  • Discussion of physiotherapy aims with you and your support network
  • Collaborative goal-setting and therapy planning
  • Thorough mobility and physical assessment
Therapy Intervention Sessions
  • Delivery of physiotherapy treatments in accordance with the established goals.
Progress Notes
  • Physiotherapist will complete documentation of each therapy intervention session.
  • This will outline assessment information and detail the treatment provided and response to treatment.
  • This allows participant progress to be monitored ongoing
Therapy Treatment Plan
  • Physiotherapists will devise a therapy treatment plan outlining participants’ goals, what therapy strategies will best help goal attainment and what measurements will be used to track this.
  • Physiotherapists at Therapy Circles Australia will update this written plan every 6 months
Exercise Programs & Resource Development
  • Physiotherapists can create exercise programs and resources tailored to your individual needs and requirements.
Support Letters
  • Physiotherapists can write support letters in the case of a change of circumstances before the end of the plan.
Plan Review Report
  • This is required every 12 months
  • Comprehensive report outlining all participant assessment findings, updated goals, therapy interventions that have been applied and clinical justification for ongoing therapy
Multidisciplinary Team Meetings (as required)
  • Liaising will all multidisciplinary team members to provide relevant progress updates, collective therapy planning and problem-solving any barriers to NDIS goal attainment that may arise.
Communication With Relevant Stakeholders Like Family & Support Workers (as required)
  • This can include email or phone correspondence to stakeholders involved in your case management
  • This is to ensure all parties have relevant and up-to-date information to best deliver care to you
Assistive Technology Sourcing & Applications
  • Physiotherapists can research, source and apply for assistive technology to help aid your physical and functional capacity.
